Acknowledgements:

Dr. Fallon thanks the individual donors, private foundations, and local (NYS Psychiatric Institute), regional (NYS) and national (NIH) organizations who over the years have contributed critical funds to make research possible.

The most generous organizations in their consistent and generous fund-raising efforts have been two tireless groups of individuals: the board of Time for Lyme Inc (TFL) (headed by Diane Blanchard and Debbie Siciliano) located in Greenwich CT and the board of the Lyme Disease Association (LDA) (headed by Pat Smith) - a national organization with chapters throughout the country that is committed to promoting research and education in Lyme Disease. These two organizations committed themselves to raising $3 million to launch a new Lyme Disease Research Center at Columbia University. Numerous individuals have made major sacrifices in time and financial resources to help make the dream of a Center devoted to Chronic Lyme disease into a reality. Their contributions are not individually listed below as their donation was through either Time for Lyme or the LDA and hence to us appears anonymous. We however acknowledge with gratitude their contribution. The commitment of TFL and LDA to finding better ways to fight the scourge of Lyme Disease has led to this collaborative effort between Columbia University, Time for Lyme, and the Lyme Disease Association. With continued help from these two organizations and other donors, sufficient funds will be obtained and the Center will be established. This will be the first Center in the United States to focus on the problem of Chronic Lyme disease and as such it will lead the country in research on this affliction.

Many others have contributed to support research as well. We wish to thank in particular major gifts from the Dever/Betz families, the Weis Monsky family and friends, the Mack family, the Sheeran Family and friends (in honor of Callean Walker Sheeran), the Lyme Disease Association, Time for Lyme, Inc, the National Research Fund for Lyme and Tick borne diseases, Turn the Corner Foundation, and the Rotary Club of Manchester Vermont.

In addition, special donations have been given to support Lyme Disease Research. We acknowledge with gratitude the individuals and businesses below who have contributed significantly to our fight against Lyme Disease:

Dr. Fallon wishes to acknowledge the many generous patients who have participated in research studies at the Lyme Disease Research Program. Without their willingness to act as co-investigators by telling their stories and be tested in objective ways, we would know very much less about chronic Lyme Disease.

Dr. Fallon wishes to thank Karen Forschner of the Lyme Disease Foundation for use of their video and educational slides (including Drs.
